Open Enrollment

People who are currently enrolled in the Diocesan Plan "BlueCross BlueShield of Michigan" will be receiving an enrollment packet from CPG (the Church Pension Group). The initial enrollment period will be approximately October 21, 2007 - November 4, 2007.  The CPG refers to this as "Open Enrollment".

If you are not currently enrolled in the Diocesan Plan, please remember that according to Diocesan Canon 8, Section 3(c) of the Diocese of Western Michigan, "each congregation that provides employee health insurance shall provide that coverage through the organization providing coverage to Diocesan employees."  If you are not enrolled in the Diocesan Plan, please contact Tammy Mazure at the Diocesan Center to receive information.

Each year there will be an Open Enrollment time period during which you can make changes to your coverage.

You may change your coverage during the year if you have a significant life change (ie, birth, marriage, divorce, loss of other coverage, etc.).  To make changes after the Open Enrollment time period, please contact the Diocesan Benefit Administrator, Tammy Mazure.

Changes and Termination

Please notify the diocesan benefits administrator Tammy Mazure within 30 days following termination of coverage.  If the termination is due to loss of employment, please indicate whether an extension of benefits is needed. The Medical Trust will bill the terminated employee directly for the extension of benefits.

Please notify the diocesan benefits administrator of address changes as soon as possible to assure carriers have correct information.
